The Impact of Language and Culture on Healthcare Delivery

These eight videos depict clinical interactions between health care providers and Spanish-speaking patients. They are designed as both a Spanish language practice tool and as case studies for discussion of critical issues and cultural nuances that can arise in a clinical setting.  Dialogue is all in Spanish; patients are all portrayed by native speakers, and health care providers by native speakers and actors with a broad range of Spanish proficiency.  The videos range from about 1 minute to 3.5 minutes in length, and for each scenario there is also a version with English subtitles.

Suggested Uses of the Videos:

  • watch the videos and post the responses to questions on Voice Threads™ in Spanish
  • watch the videos and engage in discussions on blogs or small groups related to content
  • watch the videos and have the learners develop care plans or education plans
  • use the videos as part of simulation experiences or replacement for clinical hours

Learning Objectives

Medication Management Part 1

The focus of this video scenario is on medication reconciliation, ensuring safe medication practices within the acute care setting.  The scenario portrays the process of reconciling a patient’s home medication practices with medications prescribed in the hospital.  Learners will be asked to determine the appropriateness of the medications and develop an education plan to ensure safe medication administration. 

Objectives:

  1. Develop an understanding of medication safety issues.
  2. Demonstrate the ability to reconcile medications for a newly admitted patient.
  3. Demonstrate the ability to communicate clearly and effectively with patients about their medications.
  4. Utilize Spanish language skills in video related activities (non-subtitled videos)

 

Medication Management Part 2

The focus of this video scenario is on medication management and patient education at the time of discharge from an acute care setting.  The scenario provides a foundation for discussion around effective education for a new medication.  Learners will be asked to develop an education plan around a new medication, Lovenox, in order to ensure appropriate follow up and medication management.

Objectives:

  1. Summarize medication safety issues.
  2. Develop and communicate clearly an education plan for a patient on a new medication.
  3. Utilize Spanish language skills in video related activities (non-subtitled videos)

 

Ineffective Nurse-Patient Therapeutic Interactions

The focus of this video is to provide an example of ineffective interactions among a nurse and patient.  Environmental and other contributing factors are portrayed that make the development of therapeutic interactions difficult.  Learners will be asked to describe the benefits of therapeutic relationships and identify factors that may contribute to ineffective interactions among healthcare providers and patients.

Objectives:

  1. Describe the benefits of developing therapeutic relationships with patients and families.
  2. Identify factors that may contribute to ineffective interactions among healthcare providers and patients and families.
  3. Compare factors related to misperception between cultural values of the provider and those of the patient.
  4. Utilize Spanish language skills in video related activities (non-subtitled videos)

 

Effective Nurse-Patient Therapeutic Interactions

The focus of this video is to provide an example of effective therapeutic interactions among a nurse and patient.  The exemplar provides an example of how to overcome factors in the environment that may make it difficult to interact effectively with patients and families. Learners will be asked to identify factors that may contribute to ineffective interactions, providing strategies that can be implemented to ensure effective therapeutic interactions develop among healthcare providers and patients/families.

Objectives:

  1. Identify factors that may contribute to ineffective interactions among healthcare providers and patients and families.
  2. Describe strategies that can be implemented to ensure the opportunity for effective therapeutic interactions, despite barriers within the environment
  3. Utilize Spanish language skills in video related activities (non-subtitled videos)

 

Health Care Experience Abroad

The focus of this video is to expose the learner to the challenges faced when role conflict occurs.  The video involves tensions when a student is asked to perform duties that are beyond her scope of practice by a local person who is in a position of authority.  The setting is a foreign country, adding to the level of complexity.  Learners will be able to use this video to strategize ways to prepare for and manage this type of situation.

Objectives:

  1. Discuss the challenges involved when a student is confronted with role conflict in a foreign setting.
  2. Describe strategies to employ to resolve the situation.
  3. Plan how one might prepare ahead of time for the possibility of encountering this situation.
  4. Utilize Spanish language skills in video related activities (non-subtitled videos)

 

Acute Cardiac Emergency Part 1

This video involves a patient who is experiencing an acute myocardial infarction at home.  She is from Mexico and is visiting her family when this occurs.  The first responders are present and treating her based on standard protocols. However, the patient is insistent on alternative care treatments that are specific to her cultural tradition, creating a dilemma for the first responders and her family.  Learners can use this video to gain an understanding of how to integrate alternative therapies with conventional medicine to support patient cultural beliefs and values.

Objectives:

  1. Describe the impact of a patient’s cultural beliefs and values on care delivery.
  2. Critique how the first responders managed the situation.
  3. Utilize Spanish language skills in video related activities (non-subtitled videos)

 

Acute Cardiac Emergency Part 2

This video is the second part of a situation in which a patient from Mexico who is in the United States visiting her family is experiencing an acute myocardial infarction.  She has been transported to the emergency room and is being prepped for the cardiac cath lab.  The nurse is getting report and attempting to perform an admission assessment.  Learners can use this video to gain an understanding of how to integrate alternative therapies with conventional medicine to support patient cultural beliefs and values.  This video also underscores the need to meet evidence based guidelines (e.g. STEMI) when managing patients with acute MI.

Objectives:

  1. Describe the impact of patient’s cultural beliefs and values on care delivery.
  2. Evaluate the nurse’s performance during the admission assessment.
  3. Utilize Spanish language skills in video related activities (non-subtitled videos)

 

Care Controversies at the End of Life

This video describes a difficult conversation between a physician and a patient’s family member.  In the video the physician is telling the patient’s family that their mother is terminally ill.  The family is adamant the physician not tell their mom she is dying.  The physician is very uncomfortable with this decision.

Objectives:

  1. Debate the ethical and legal implications of the family’s wishes.
  2. Discuss how different cultures balance a patient’s right to medical information and privacy with family involvement in care.
  3. Utilize Spanish language skills in video related activities (non-subtitled videos).
